Description of Services: Assists clients to make healthy choices and decisions * Offers educational workshops, leadership training and support to help increase self management and personal skills.

Offers the following services both on-site and off-site:

  • fall prevention
  • personal safety
  • pedestrian safety
  • nutrition awareness
  • healthy lifestyles
  • wellness clinic

The Stanford University Licensed six week Self Management Programs include:
  • Chronic Pain Self Management
  • Diabetes Self Management
  • Chronic Illness Self Management

By appointment wellness clinic also offers one on one consultation pertaining to personal health concerns such as blood pressure levels, blood sugar levels and diet.
