Community Psychogeriatric Outreach Teams (CPOT)
These outreach teams offer a variety of psychogeriatric services and assessments by multidisciplinary teams. Some of the clinics can assist in cases of seniors who are home-bound.

1. Baycrest Geriatric Psychiatry Community Service (GPCS) - Offers assessments, treatment by multidisciplinary teams and long-term follow up.
2. Sunnybrook Health Sciences Centre - Offers assessment and treatment by multidisciplinary teams including long term follow up.
3. North York General Hospital - Offers mental health services to address the unique needs of older adults.
4. Providence Healthcare - Provides assessments, treatments and short/long-term follow up.
5. West Park Healthcare - Offers assessments by mental health professionals, review of health challenges and medications, provides advice and information and periodical follow up.
6. Centre for Addiction and Mental Health (CAMH) - Offers assessments and psychogeriaric consultations, counseling, education, medication monitoring and home visits for follow-up.
