Fees: Funded services, no cost to client.
Eligibility - Population(s) Served: Must be at least 16 years of age or older with a permanent physical disability and/or acquired brain injury, ability to direct care, and OHIP coverage.
Application: Self, family, or advocate, completion of Ontario March of Dimes Service Application, and Applicant Interview.
