Description of Services: Programs to reduce social isolation, promote social connection, and improve the well-being of housebound individuals in Toronto’s downtown core. Recruits, screens and matches volunteers with clients for:
  • Friendly Visiting Program
  • Assistance for Medical Appointments and Grocery Shopping
  • Telephone Reassurance Program
  • Information and Referral

Fees: Free
Eligibility - Population(s) Served: Isolated adults 65 years of age and older, and adults of 18 years of age and older with physical disabilities * live independently or with an informal caregiver in their own homes * medically stable with current support from health care professionals * physically independent with assistive devices and aids * only available for people who require on-going support
Application: Professional client referral required; each client assessed by Society staff; follow-up visit by staff; self-referrals accepted
