Eligibility - Population(s) Served: | Isolated adults 65 years of age and older, and adults of 18 years of age and older with physical disabilities * live independently or with an informal caregiver in their own homes * medically stable with current support from health care professionals * physically independent with assistive devices and aids * only available for people who require on-going support | ||
Application: | Professional client referral required; each client assessed by Society staff; follow-up visit by staff; self-referrals accepted | ||
