Helps individuals transition back into the comfort of their own home after a brief or extended stay at a local hospital * help coordinate the support services individuals may need during this time of transition PROGRAM HIGHLIGHTS

| Fees: | Free for qualifying patients being released from participating hospitals. The Ministry of Health covers all costs for this service. There are no charges for patients participating in the program. Patients only pay for any groceries and prescriptions that are picked up. | ||
| Eligibility - Population(s) Served: | Seniors 65 and up * older adults ages under 65 with related physical disabilities leaving the hospital * A client must be able to ambulate and require one-person assistance | ||
| Application: | Hospital staff such as GEM Nurse, Discharge Planner, Social Workers or Nurse In-charge can make a referral | ||
