Description of Services: Care Navigators conduct eligibility assessments, including the InterRAI Community Health Assessment (CHA), and work collaboratively with seniors and their caregivers to develop individualized care and service plans that reflect their needs, goals, and preferences * provide home visits, safety checks, and ongoing case management, offering support with healthcare system navigation and access to appropriate community resources * through the Client Intervention and Assistance Service, Care Navigators also provide advocacy, information, and referrals for vulnerable or at-risk older adults to help promote safety, independence, and overall well-being
