| Description des services: | An outpatient service adults over the age of 65 with complex medical and psychosocial issues such as depression, anxiety and other mental health illnesses. The interprofessional services include: - Comprehensive assessment and diagnosis, - Treatment options, including medication management and non- pharmacological approaches (e.g. psychotherapy, relaxation and breathing techniques) - Problem solving therapy and goal setting - Group-based counselling and therapy - Group interventions, activities and exercises | ||

| Procédé et formulaires: | Referrals from a physician or nurse practitioner for this service can be initiated through Baycrest Central Navigation. The Central Navigation team will assist community agencies, primary care providers, and other healthcare professionals in navigating Ambulatory Services at Baycrest. Paper-based and online referral options are available on the Ocean Referral Network. | ||
