Program: Support Within Housing - Toronto - Mutual Street

Organization: WoodGreen Community Services
Description of Services: Permanent affordable housing for individuals recovering from mental health challenges who have experienced homelessness. Support services include case management, trauma care, living standards coordinator and two community development workers who coordinate recreation opportunities and external collaboration with care partners. Some units are accessible and some are designed for hearing-impaired clients. Average length of stay is over 10 years.

Application: Referrals are accepted from anyone, including self-referrals. Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment. Once referred to Woodgreen, the housing manager will arrange an initial interview, followed by a tour or the residence for acceptable applicant. Eligibility Criteria: Individuals who have experienced homelessness and are recovering from mental health challenges. Must be able to live independently with a low to medium level of support. Individuals may also contact WoodGreen Housing and Homelessness Services.
