Description of Services: Services and programs to meet the needs of youth and young adults with cerebral palsy or acquired brain injury as they transition from pediatric to adult rehabilitation services * provides a single point of access to specialized rehabilitation while helping patients learn how to manage their own care and navigate the adult system

Services may include:
  • nutrition counselling
  • spasticity management
  • pain management
  • speech language therapy consultation
  • mobility assessment and supports consultation
  • consultation around exercise programs
  • short-term supportive counselling
  • independent living skills development

Fees: Most services covered by OHIP
Eligibility - Population(s) Served: Youth 16-18 years with cerebral palsy or acquired brain injury who are transitioning from Holland Bloorview Kids Rehabilitation Hospital to adult rehabilitation
Application: Medical referral required from Holland Bloorview Kids Rehabilitation Hospital
