Program: Specialized Geriatric Services

Description of Services: Specialized services offered through the emergency department, in-patient units, outpatient clinics and in the community for the elderly * provided by a multidisciplinary team * member of the Regional Geriatric Program of Toronto
 
Falls Prevention Clinic Provide consultation with a geriatrician and physiotherapist to assess adults over the age of 65 who experience falls or at risk for falls * six-week group exercise program * education sessions about home safety, home exercises and safe medication practices
 
Geriatric Day Hospital (Maintenance) Hospital-based outpatient day program with a focus on geriatric rehabilitation * services provided by a geriatrician, nurse clinician, occupational therapist, physiotherapist, recreation therapist, social worker, and speech language pathologist
 
Geriatric Emergency Management Consultation by a nurse specialized in older adults within a hospital emergency room to provide assessment, diagnosis, identification of  frail and at risk elderly, and to initiate appropriate treatment in the hospital or community
 
Geriatric Outpatient Clinic (Maintenance) Consultative assessment provided by a geriatrician * works in collaboration with patients' primary care physician and/or primary care providers
 
Geriatric Outreach Team (Progression) Assessment of homebound older adults who have complex issues related to health, daily functioning, and social concerns * provides recommendations to improve patient's level of health, functionality, and independence * team includes a nurse clinician, occupational therapist, and physiotherapist
 
Geriatric Psychiatry Inpatient Program Provide hospitalization for a short-term to assess and treat older adults with acute severe  mental health disorders
 
Internal Inpatient Geriatric Consultation Teams Comprehensive assessment by internal consult team * provide recommendations on older patients at Sunnybrook who have complex medical and functional issues * team composed of a geriatrician and two registered nurses
