Description of Services: | This program offers one-on-one case management alongside safe, affordable and secure housing for older adults living with mental health and substance use * Together with a case manager, under a harm reduction framework, older adults work towards independent living and towards better health, social and economic outcomes. | ||

Eligibility - Population(s) Served: | Older adults, aged 55+, with problematic substance use/gambling or concurrent disorders, who are homeless/marginally housed, have no legal concerns and willing to work on a recovery plan with a Case Manager. | ||
