Eligibility - Population(s) Served: Infancy & Early Childhood Program: ages 0-6 with a confirmed or suspected diagnosis of developmental delay
Clients of all ages with a confirmed diagnosis of an intellectual disability
All clients live in the city of Toronto and have a postal code that starts with 'M'
All clients must be referred by a SLP or OT * the referral form and eligibility is on the website under specialized programs
Application: Contact Intake by calling 1-833-575-5437. Referring Speech Language Pathologist or Occupational Therapist faxes ACWA referral to 416-925-3402.
