Description of Services: House Calls improves the health and quality of life of homebound seniors by providing inter-professional healthcare at home to those who have difficulty accessing typical office-based primary care. House Calls services are covered under OHIP and require a valid health card

Eligibility - Population(s) Served: People of 65 years or older who have difficulty accessing a family physician because of physical, cognitive or social frailty
Application: Send referral form through fax or call to complete intake for assessment
