Description of Services: Referrals to and provision of services for older adults seeking home and community care supports to stay independent * team may connect clients with inpatient and outpatient options, home care and support services, or specialized geriatric services, as well as help client navigate health care system and access caregiver support * team includes a nurse, social worker, pharmacist, geriatrician, and an Ontario Health atHome Care Coordinator

Eligibility - Population(s) Served: Adults 65 years and older who sense a decline in function at home and would like to improve, are interested in home and community supports to stay independent, or are homebound and would benefit from specialized geriatric care in the home * must live in catchment area
Application: Medical referral required
