Program: Young Carers Program

Organization: Hospice Toronto
Description of Services: Supports children and youth (5-18) who have assumed a significant caregiving role for a family member living with chronic or terminal illness, a disability, a mental illness or addiction, or a language barrier. YCP provides therapeutic, educational and recreational programs for Young Carers. The programs provide Young Carers a safe space to explore their caregiving role, develop skills, foster resilience and growth, and take a break from their caregiving role.

Eligibility - Population(s) Served: Families Only ; Youth Only ; Caregivers Only ; Children Only
Application: Referrals are accepted from: self, family, friend, school, agency, physicians. A referral to the program must be completed by filling out the referral form found on the website. Once the form is completed it may be faxed or emailed. Please call if you require assistance with the referral form. Eligible families will be contacted to set up an intake meeting to assess the needs of the young person and their family.
