Eligibility - Population(s) Served: | Adults aged 55 years or older leaving hospital emergency department or inpatients units, without support from friends or family upon discharge, and able to direct their own care | ||
Application: | Must have member of professional care team in hospital to complete a Home At Last referral form. For referral form, go on website | ||
