Description of Services: This program operates on a 'housing first' model, assisting people with HIV/AIDS and a history of homelessness, substance use, and frequent use of inpatient and emergency services. The program provides intensive case management in a formal partnership with Fife House who provide the 32 single-dwelling housing units in the private market with rent subsidies.

Eligibility - Population(s) Served: 2SLGBTQI+ Only ; Homelessness / At Risk Only ; Concurrent Disorders Only ; HIV/AIDS Only
Application: Referrals are accepted from anyone, including self-referrals. Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
