Description of Services: Helps seniors and their caregivers navigate the health care system ensuring they are connected to the care and services that are needed and wanted * support when seniors are faced with life changes and significant care needs, including:
  • referrals to community resources and programs
  • client advocacy
  • crisis intervention
  • help with long-term care options

Education and information sessions include:
  • wellness sessions
  • counselling and tours

Fees: Free
Eligibility - Population(s) Served: Older adults, primarily from the Japanese-Canadian community, who need physical, social, or emotional supports
