Program: Caregiver Support

Organization: Senior Persons Living Connected
Description of Services: Individual support, training and education sessions for caregivers on topics such as caregiving management, advanced directives, safety, palliative care, elder abuse, mental health and addictions, dementia, chronic illness and bereavement.

One-on-one counselling and case management which may include service advocacy, linkages with community resources and identification of needs for caregivers.

Support services available for caregivers such as personal care, homemaking, respite services, transportation, Meals-on-Wheels, friendly visiting and adult day programs.

Monthly support groups available for caregivers in English and Cantonese.

Support groups are delivered in hybrid format (participants can choose to attend virtually or in-person).

Fees: Individual caregiver support, training, education and support groups are free. Nominal fees may apply for some support services such as personal care, homemaking, respite services, transportation, meals on wheels, friendly visiting and adult day programs.
