In-home service provided to eligible inidividuals currently residing at St. Paul's L'Amoreaux Centre building with 24-hour on-site personal care and support services to assist frail older adults 65 years and older and adults with physical, functional or cognitive concerns Eligibility for clients to receive supportive housing services is determined by the inter RAI Community Health Assessment * health care professionals, both internal and external, can make referrals for service initiation * limited supportive housing client spaces available annually Services include:
