Description of Services: In-home service provided to eligible individuals currently residing at Wishing Well Manor building with daily on-site personal care and support services to assist frail adults aged 65 years and older with physical, functional or cognitive concerns

Eligibility for clients to receive supportive housing service is determined by the interRAI Community Health Assessment * health care professionals, both internal and external, can make referrals for service initiation * limited supportive housing client spaces available annually

Services include:
  • safety reassurance
  • assistance with personal care
  • light homemaking and laundry
  • light meal preparation
  • medication reminding
  • socialization
  • case management
  • supervised exercise

Fees: Free
