Program: Ride Connect Transportation

Organization: CANES Community Care
Description of Services: Non-emergency transportation to local medical appointments and regular activities such as group shopping and religious or cultural events
  • rides to be requested minimum 72 hours in advance
  • rides will be confirmed 24 hours before the client is picked up
  • pick up times may vary, and there may be a wait time to assist requests for older adults
  • any cancellation will require the clients to notify the office 24 hours in advance

Fees: Subsidies available for those who qualify
Eligibility - Population(s) Served: Adults 55 years or older or persons with disability * if the client is travelling with a caregiver, there is no extra charge for the trip * if a passenger does not have someone to travel with, CANES can provide a caregiver to the client for passenger assistance during their ride
Application: Call * no referral required * new clients will be assessed on the phone, which takes about 5 minutes
