Description of Services: Outpatient Services

Services offered in clinic setting and in the home when patients are no longer physically able to come to the clinic
  • complex pain and symptom management
  • support with establishing goals of care and advanced care planning
  • support with addressing one’s physical, spiritual, emotional, social and informational needs
  • support for patients and families facing difficult end of life decisions such as withdrawal of treatment
  • procedures such as paracentesis and blood transfusions
  • seamless transition from inpatient care and direct admission wherever possible
