Neighbourhood Link Support Services
Description of Services: Supports for older adults, and adults with disabilities * helps individuals remain independent in their own homes for as long as possible * referral to other community services
 
Adult Day Program Adult Day Program * social, recreational, and therapeutic activities for clients with dementia provided in a group setting designed to focus on, and retain, remaining strengths  in a safe, and secure setting
 
Client Intervention and Assistance Client intervention and assistance for people facing homelessness, critical life changes, abuse, or isolation * referrals to other community support services as necessary
 
Community Programs and Congregate Dining (SALC) Social activities and a hot meal * activities can include games, crafts, trips and physical activities
 
East Toronto Seniors Centre Community/recreation/wellness centre for older adults * air conditioned
 
Friendly Visiting Regular volunteer visits and/or phone calls to seniors and people with disabilities who live alone, to provide companionship and reassurance. Must have VSS check and references
 
Meals on Wheels Program Hot or frozen meals delivered Sun-Fri * second meal can be delivered on Friday for Saturday * also meals for relief of caregiver. New mothers can access the service up to six weeks.
 
Security Check Daily telephone calls and medication reminders made to seniors to ensure they are safe in their own home
 
Social and Recreational Activities - Chinese Programming Social, recreational, and educational activities in Cantonese and Mandarin * may include recreational games, monthly luncheons, music, art classes, day trips and special event celebrations * monthly newsletter in Cantonese or Mandarin for participants
 
Transportation In partnership with Toronto Ride- provides Non-emergency transportation services within the GTA for medical appointments, grocery shopping, banking, social programs and other errands * service provided in partnership with Toronto Ride
